| To apply to GPAF for financial aid,
please complete our Income/Expense Application Form, and mail it
to GPAF at the address printed on the Form.
We regret that we are unable to accept applications via the Internet.
We require a manual signature on the Form for our records.
The Form can be completed in one of three ways:
- Fill out the Form found at this link,
and use the "Print" button at the bottom of the Form
to print the Form. Please note that you cannot save your
entries; once you close the Form web page your entries will
be deleted. You may also print the blank Form, and fill
it in by hand.
- Download a Microsoft Word version of the Form. You can
then either fill it out in MS Word and print it, or print
it and fill it out by hand.
